Every person’s journey is different. Your responses will help us get to know you, understand your unique story, and prayerfully discern how we can best accompany you during this time.

Why do we ask about marriage?
The Catholic Church takes marriage seriously and recognizes it as a lifelong bond. Because a current or previous marriage may affect someone’s path toward receiving the sacraments, we ask everyone these questions early in the process.
This isn’t meant to judge you or disqualify you from OCIA. It simply helps us understand your situation, support you well, and determine whether an additional Church process may be needed. You’re welcome to write, “I’d prefer to discuss this privately,” wherever needed.
Your information will be kept confidential
Everything you share will be treated with care and kept confidential. Your responses will be seen only by the pastor, the Director of OCIA, and, when necessary, another priest assisting with your formation.
There’s no pressure or obligation to become Catholic simply because you complete this form. OCIA is a place where you can ask honest questions, encounter Jesus Christ, and prayerfully discern where He is leading you.
